It is your turn to develop an audit tool to assess a medical record or for conducting observational rounds (examples: hospital floor, Nursing Home Unit, Clinic, Urgent Care Center). Criteria to be included would be the title of the audit tool, date, name of the auditor, and you determine the rest.
